    Extras (engl.: Perk, pl.: perks) sind Fähigkeiten oder besondere Extras, die der Protagonist bei Stufenaufstieg oder durch das erfüllen besonderer Voraussetzungen erlangen und ihn anschließend dauerhaft beeinflussen kann. Anders als in den Vorgängern (Fallout und Fallout 2), kann der Held bei jedem Stufenaufstieg ein Extra wählen oder steigern. Einige Extras wie Schwarze Witwe/Ladykiller oder Kind im Herzen, erweitern gezielt die Sprachoptionen des Spielers, es kann jedoch bei gewissen Quests/NPCs vorkommen, das man durch ein bestimmtes Extra eine neue Sprachoption erhält. (Da diese Extras im Grunde nicht dazu gedacht, sind die Sprachoptionen zu erweitern und solche Begegnungen selten sind, wird dieser Effekt beim entsprechenden Extra nicht beschrieben, betrachtet es einfach als kleines Zuckerbrot wenn sich euch eine solche Sprachoption eröffnet).
